Building Systems for Quality Health Care

Last Mile Health saves lives in the world's most remote communities by partnering with government to bring essential primary healthcare services to the doorsteps of people living at the last mile. Together with the Government of Liberia, it supports scale of a national community health worker (CHW) program that – by 2021 – will ensure consistent, predictable, and timely access to lifesaving care for all 1.2 million people living in the country’s most remote and hard to reach areas.

With over 3,100 CHWs already deployed, Last Mile Health remains committed to cultivating a workforce of skilled primary healthcare leaders who are passionate about improving community health, as well as building the systems needed to overcome last mile barriers to access to care. Recognizing that management and supervision are critical to program quality, CHW motivation, and ensuring integration within the pre-existing health system, Last Mile Health has developed strong support structures for CHWs and their supervisors.

Eyea Mulbah is one of 11 Quality Assurance Officers who provide training, support, and leadership to those delivering community health services - including frontline CHWs and the facility-based clinical supervisors deployed to oversee them. After training as a nurse and working in a health center, Eyea joined Last Mile Health upon realizing that the people most in need of health care are not always the ones able to access it, and that to reach those with the greatest need, you have to go to the most remote areas of Liberia. “Very few patients that come to the hospital are from the remote areas, so you have to build their interest from the community level”, says Eyea. And so began her lifelong commitment to strengthening primary healthcare systems at the community level.

Eyea moved from her home in Lofa County to Rivercess County to support the CHW program, and three days each week travels by motorbike, on foot, and in canoes to supervise community health professionals in the most remote areas. “[In] one of the catchment areas you have to walk nine hours to reach the community. Or you have times where you have to cross a swamp to reach the community. The distances are very challenging, but that is why Last Mile Health is here.”

By creating strong, integrated structures that support CHWs to provide quality, lifesaving community-based health care, Last Mile Health is building systems that prevent death and suffering in areas previously deemed as too difficult to serve. When asked her motivation for serving the last mile, Eyea says, “a building cannot stand without its foundations. And if health should improve in Liberia, it has to be rooted in the most remote areas.”
