Summary

This project will recruit and hire a Community Health Connector to be a liaison with the Glenwood, Millville and St. Andrews targeted neighborhoods to help them connect with online services, includin but not limited too completing the 2020 Census, job applications, public assistance forms, community participation virtual programs, etc.

Challenge

As Covid-19 has resulted in more people sheltering in, especially those who are most vulnerable, more services and programs are being delivered virtually. A Summer 2020 survey of the targeted neighborhoods served by the LEAD Coalition revealed that 20% of them cite communication as an urgent need.

Solution

The Community Health Connector will solve the problem by being able to help citizens navigate the virtual and secure the resources and/or services they need in order to lead better lives.

Long-Term Impact

The Community Health Connector will help 20 to 40 people weekly to get what they need in order to be healthy amidst the Covid-19 pandemic.
