In the rural district of Kyenjojo, Uganda, a remarkable transformation has taken place in the life of six-year-old Michael. Living with his grandmother and uncles, Michael's story demonstrates how comprehensive community-based support can create lasting positive change for children with complex needs.
Michael's journey began in 2019 when he experienced his first seizure, an event that dramatically altered his life. Following hospitalisation, his family noticed significant changes and delays in his development. By the time he was referred to our community disability centre in January 2021, Michael was severely malnourished, experiencing multiple daily seizures, and struggling with basic functions like eating and communicating.
The initial assessment revealed the interconnected challenges Michael faced: severe acute malnutrition (SAM), uncontrolled epilepsy, and significant developmental delays. He could not recognize familiar faces, had difficulty eating, and showed minimal interest in his surroundings. Our team knew that addressing his nutritional status was crucial before any other progress could be made.
We began Michael's recovery journey through a coordinated approach that combined medical care at Kyenjojo General Hospital with community-based support. His grandmother received extensive nutrition education, focusing on nutrient-dense meals and proper feeding techniques. The family also participated in our agriculture support program, establishing vegetable gardens to provide sustainable access to nutritious food.
The results have been transformative. From a critically low weight and MUAC (Mid-Upper Arm Circumference) reading in the red zone, Michael has now achieved and maintained a healthy nutritional status, with his weight increasing to 25.6kg and his MUAC reaching 18.5cm. More importantly, this nutritional improvement has supported his overall development.
Today, Michael shows remarkable progress across multiple areas. His seizures are under control, he can feed himself, and he's beginning to communicate through gestures. He recognizes familiar faces, explores his environment with support, and responds to simple instructions. Perhaps most heartwarming is his grandmother's report that he now expresses joy, especially at mealtimes, showing appropriate emotional responses through laughing and crying.
However, challenges remain. Since the family's move to Kyegegwa, the long distance to our outreach services has made regular attendance difficult. This highlights the ongoing need for expanded community-based services to reach more children like Michael.
Michael's story illuminates the critical role of comprehensive, community-based nutrition services in supporting children with multiple disabilities. When we address a child's nutritional needs within their community context, we create a foundation for broader developmental progress. His journey shows that with proper support, children facing multiple challenges can make significant strides in their development and quality of life.
