Saiban-e-Maa ("shelter for mothers") is a Sindh Ajrak Welfare Trust initiative establishing small, 3-bedded maternity homes in rural union councils. Each provides free antenatal, skilled delivery, emergency response, postnatal care, and referrals. Annual target: 5 homes, beginning with a 3-home Year 1 pilot to validate the model before scaling across Sindh.
Sindh's maternal mortality ratio exceeds 220 per 100,000 live births, higher in rural areas. Only two in five births have skilled attendants. Barriers: no nearby facility (1-2 hours away), shortage of skilled birth attendants, unaffordable private care, delayed emergency referrals, and weak antenatal follow-up. This causes preventable maternal deaths, stillbirths, and newborn complications in impoverished communities.
Saiban-e-Maa places a small, permanently staffed maternity home within each community. It offers 24/7 skilled birth attendance, free antenatal risk screening, basic emergency obstetric response, and a functioning referral pathway to district hospitals. The 3-bedded, "cookie-cutter" design keeps costs low and enables rapid replication across multiple union councils, ensuring care is reachable on foot or short transport.
Reduces maternal and neonatal mortality by shifting unattended home births to skilled facility deliveries. Strengthens antenatal coverage and early high-risk detection. Eliminates catastrophic health spending. Builds community trust in formal care, shifting away from traditional dais. Creates a replicable low-cost model scalable across Sindh. Generates local clinical jobs for women. Sustained at 5 homes/year, reaches 25 communities within five years.
