Summary

CEDEP's drop-in centres are Malawi's only dedicated LGBTIQ+ clinics, providing essential HIV care and support in a safe, welcoming environment for 7,000 individuals. These clinics remain a vital lifeline as funding cuts threaten services and public facilities await the guidelines and data systems needed to ensure a safe transition and continuity of care. CEDEP needs to ensure LGBTIQ continue to access health services as the government establishes friendly centers for a successful transition.

Challenge

Funding cuts are pushing CEDEP to transfer LGBTIQ+ clients to public facilities before safeguards are in place. Stigma remains a barrier, while transition guidelines, a national database and an inclusive integration model are still pending. Premature transfers could disrupt HIV services. Continued funding is urgently needed for CEDEP to maintain safe, stigma-free services while supporting a coordinated and sustainable transition to public facilities.

Solution

The project will maintain accessible, stigma-free HIV services for LGBTIQ+ clients while strengthening readiness for a safe transition to public facilities. CEDEP will provide continued service delivery, client referrals and follow-up, engage the Ministry of Health to develop transition guidelines and inclusive service models, and strengthen public facility capacity to deliver LGBTIQ+-responsive care. This will prevent service interruptions and ensure a coordinated, sustainable transition.

Long-Term Impact

The project will secure long-term continuity of inclusive HIV services for approximately 6,000 LGBTIQ+ clients currently reached by CEDEP, including 600 clients in care expected to transition to public facilities. By strengthening public-sector capacity, establishing inclusive transition and referral systems, and reducing stigma, the project will help embed LGBTIQ+-responsive HIV services within the national health system, ensuring clients can access safe, quality care beyond the project period
