More money, prevention and less repression of populations at risk: these messages will be hammered home from Monday in Amsterdam during the International AIDS Conference, to avoid a rebound in this epidemic which has caused 35 million deaths .experts and activists — are expected from Monday to Friday at this high mass, which takes place every two years.

Today, 36.9 million people live with the HIV virus, hoping that it does not worsen into AIDS. Nearly three in five take antiretroviral treatments to prevent it, the highest proportion ever reached. The number of infections is falling and for the first time since the turn of the century, the annual death toll fell below one million in 2016 (990,000) then 2017 (940,000).

“Loss of control of the epidemic”
But paradoxically, this progress leads to a relaxation in prevention which, combined with a drop in international funding, raises fears of a rebound in the epidemic. “The last time I spoke here, in 1992, I never thought I would return twenty-six years later, alive and well,” David Barr, an American HIV-positive activist, said on Sunday. during a symposium organized on the eve of the Conference. But this success is “incredibly fragile”, he warned, fearing that we would return to “the horror of 1992”, with a surge in infections and deaths.

In the Political Declaration adopted at the high-level meeting on HIV in June 2021, the community
international community reaffirmed its commitment to achieving target 3.3 of the Sustainable Development Goals
consisting in particular of ending the AIDS epidemic by 2030, and adopted quantified intermediate objectives
for 2025, and in particular to reduce to less than 370,000 the annual number of new HIV and
less than 250,000 the annual number of AIDS-related deaths by 2025 or reach the 95-95-95 target in terms of
screening, treatment and inhibition of viral replication in all demographic categories, in
all groups and in all geographic contexts.

Around 2.9% of Burundi's population is HIV positive, a figure likely underestimated. Through several projects, the AFD group supports on site the activities of Save Life Make Difference SALIMADI via its global Giving system, but also thanks to L'Initiative, a program to fight against pandemics financed by the Membres of donation to GlobalGiving

Raise awareness and screen as many people as possible

To stem the HIV epidemic raging in the country, beyond treatment, prevention and screening are essential. The ANSS therefore carries out activities in schools, in communities, and encourages everyone to go and get tested.

Following the effects of the civil war which broke out in 1993 and lasted more than twelve years, health indicators saw a sharp decline compared to previous years. Indeed, for example, life expectancy at birth fell from 51 years in 1993 to less than 42 years in 2005. Infant mortality was 800 deaths per 100,000 births in 2012. The HIV prevalence rate / AIDS is 9.5% in urban areas and 2.5% in rural areas. The overall vaccination coverage rate of 80% in 1992 fell to 55.3% in 1997 and was 78.6% at the end of 2015. Childbirth assisted by qualified personnel was at 34% while contraceptive prevalence at 8% in 2015.

It is also worth mentioning the quantitative and qualitative insufficiency of health personnel, especially at the peripheral level because 50% of nurses and 80% of doctors worked in Bujumbura, the capital. Alongside these indicators, the chronic underfunding of the health system and other aggregates, notably the availability of medicines, hygiene in healthcare environments and others, have resulted in a significant deterioration in the quality and accessibility to care, especially for the poor and vulnerable! A more than alarming situation!

Another not least challenge concerns malnutrition. Mothers carrying in their arms the bodies of their very weakened and emaciated children are sitting in the nutrition service corridor at Muyinga hospital. They are there two weeks or even three without improvement. Visibly very discouraged, the mothers are asking for substantial support from the State to meet their needs.

Note that the rate of acute malnutrition in Burundi is 6%. Underweight is still high (29%), close to the critical threshold of 30%. The 2023 EDS results showed that chronic malnutrition (stunted growth) remains a very serious problem in the country, exceeding the critical threshold of 40%. A lot of effort must therefore be made to eradicate the scourge because in fact, hunger weakens man before killing him!!

As we can see, progress has been made but major challenges exist, particularly in terms of the quality of care, hygiene in general and the management of biomedical waste, malnutrition, etc. Indeed, regarding the quality of care, in certain health facilities, notably in Kayanza, it has been noted that two to four children share a single bed in the maternity ward. The mothers, sick guards, have to take turns all night. They cannot sleep and often end up getting sick from fatigue!! Besides this, the risk of contamination is very high.

No single prevention method or approach can stop the HIV epidemic alone. Several methods and interventions have demonstrated high levels of effectiveness in reducing the risk of HIV infection and protecting against HIV infection, including male and female condoms, administration of antiretroviral drugs as pre-exposure prophylaxis (PrEP), voluntary medical male circumcision, behavioral change interventions to limit the number of sexual partners, the use of clean needles and syringes, opioid replacement therapy (e.g. methadone) and treatment of people living with HIV with the aim of reducing viral load and preventing further transmission of the virus.

Despite the availability of this growing range of effective HIV prevention tools and methods, as well as a massive expansion of HIV treatment in recent years, the number of new infections has not declined sufficiently. globally among adults. The goal of the 2016 United Nations Political Declaration on Ending AIDS is to reduce the number of new HIV infections from more than 1.8 million in 2016 to fewer than 500,000 by 2020.

Three interconnected reasons appear to explain the failure to implement effective programs at scale: lack of political commitment and, consequently, inadequate investments; reluctance to address sensitive issues regarding the sexual and reproductive needs and rights of young people, key populations and harm reduction; finally, lack of systematic implementation of prevention, even when political environments allow it.

UNAIDS works to stimulate leadership and accountability on HIV prevention at global and national levels, as well as to accelerate the implementation of effective HIV prevention programs at national level, by providing guidance on effective approaches to achieve the prevention goals of the 2016 Political Declaration, which include access to combination prevention options, including condoms, PrEP, harm reduction and voluntary medical male circumcision, for at least 90 % of people at risk by 2020, especially young women and girls in high-prevalence countries and key populations, coverage of 3 million people at high risk with PrEP, coverage of 25 million of men with voluntary medical male circumcision and the distribution of 20 billion condoms in low- and middle-income countries.

The Executive Director of UNAIDS has called for the establishment of a new global prevention coalition and the establishment of a road map for 2020. The objectives of this coalition are to: create a platform trains policymakers, program managers and civil society representatives on HIV prevention to strengthen political commitment; define critical milestones and targets to ensure effective scale-up of prevention programs as part of a global prevention roadmap; and improve accountability of all stakeholders and technical support for HIV prevention programs, in support of achieving the objectives and commitments of the 2016 Political Declaration on prevention.

We hope we have sufficient funds to start our project, help patients living with AIDS, and teach what does not carry this disease.
