More than 11.5 million people are incarcerated worldwide each year, and millions are released back into their communities with ongoing healthcare needs. TUNMAPS bridges this gap by linking them to health facilities, providing referrals, follow-up and retention support to ensure continuity of HIV, TB, mental health and other essential care. Your donation will connect former prisoners to timely, continuous uninterrupted healthcare, helping them stay in care and improve their health.
In Uganda, about 110,000 people are released from prison annually yet healthcare support often stops at release. Many are not referred, linked to community health facilities or followed up, leaving people with HIV, TB, mental health and other conditions at risk of treatment interruption. Weak prison-community referral systems, stigma, poverty and limited reintegration support increase loss to care. Therefore, TUNMAPS intends to bridge this critical gap starting in Greater Masaka and spread
TUNMAPS will bridge the gap between prison and community healthcare by conducting prison visits, pre-release health screening and counselling, and preparing referrals before release. After release, beneficiaries will be linked to health facilities and supported through appointment reminders, home visits, nutritional support and follow-up. TUNMAPS will coordinate with health facilities to track referrals, identify missed appointments and reconnect ex-offenders to care, retention and continuity.
The project will improve long-term continuity of healthcare for at least 500 newly released prisoners in Greater Masaka and Southwestern Uganda. By establishing stronger referral and follow-up links between prisons, health facilities and communities, beneficiaries will be more likely to remain in HIV, TB, mental health and other essential care. This will reduce treatment interruptions, loss to follow-up and preventable illness, while providing a scalable approach for post-release healthcare
