There are thousands of older adults in Puerto Rico at imminent risk of experiencing homelessness. Most of them face social determinants of health that expose them daily to losing their homes, such as economic stability, education access, and quality, health care access and quality, neighborhood environment, and social and community context, among others. Our Community Development Program impacts more than 1,000 older adults in Puerto Rico to prevent them from becoming homeless.
There are thousands of older adults in Puerto Rico at imminent risk of experiencing homelessness. Most of them face social determinants of health that expose them daily to losing their homes, such as economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context, among others. Our Community Development Program impacts more than 1,000 older adults in Puerto Rico to prevent them from becoming homeless.
The Community Development Program's staff visits more than 80 communities within and outside of the greater metropolitan area of San Juan to provide direct services to the elders. Our services coordinator, case manager, and social workers visit those communities to impact older adults with immediate services that fulfill their needs, empower community leaders, and provide tools so that individuals and communities can obtain their highest possible level of self-sufficiency.
The project will help the impacted communities to become stronger and more resilient and their older residents to be more empowered to access services and develop skills to avoid homelessness with direct services and the maximization of available resources such as other partner organizations, government agencies, the private sector, community leaders, and any other stakeholder.
