Roots Community Health recently transformed our Turning Point Program into a transitional housing for chronically ill unhoused people. This program provides temporary, but stable housing while equipping its participants with access quality health care to improve their health outcomes. Additionally, it connects them to Roots' expansive cache of services providing physical, emotional, and behavioral care to them, ensuring their health does not impede their ability to thrive.
Oakland's unsheltered population has jumped nearly 50% in the past two years and there are now over 4,000 people living on the streets. Those who are sick and disabled will be prioritized for housing, while those who just need a hand to get back on their feet will be overlooked by these housing solutions - until they, too, are sick and disabled. We need more solutions that can help people before it's too late.
Turning Point Program equips homeless individuals with housing and quality health care, coupled with mental health services and wraparound support, facilitating a smooth transition to a stable, self-sustaining life. We are demonstrating an effective, replicable model for ending homelessness.
We are determined to help solve the persistent social problem of homelessness, implementing solutions that address the root causes of the problem. Roots has a long history of addressing social determinants of health disparities often not prioritized by traditional medical providers.
