Project Heart will help individuals and families in Nevada and Placer counties, California, to move through recovery, reentry, housing instability, and other destabilizing challenges. Through peer mentorship, weekly groups, housing and reentry navigation, transportation assistance, community service, and connections to essential resources, participants gain the practical support, accountability, and belonging needed to rebuild stable lives.
People working to rebuild their lives often face several barriers at once: unstable housing, unemployment, transportation challenges, fractured relationships, substance use, or involvement with the justice system. Even when someone is ready to change, navigating these challenges alone can lead to isolation and setbacks. Our rural communities need consistent, relationship-based support that treats each person with dignity and stays present through the difficult work of rebuilding.
Project Heart meets people where they are and walks beside them. Peer mentors and staff provide trusted relationships, practical guidance, weekly groups, housing and reentry navigation, transportation assistance, community service opportunities, and connections to treatment and other resources. This steady presence helps participants take the next achievable step, remain connected during setbacks, and build the stability, accountability, and confidence needed to move forward.
Recovery becomes sustainable when people have more than short-term help. They need stable relationships, practical support, purpose, and a place in community. Participants can move from crisis toward housing stability, employment, healthier relationships, and community involvement. Shawn's journey from receiving support to becoming an employer shows the ripple effect: when one person rebuilds, families and the wider community grow stronger, and that person can create opportunity for others.
