Project Results (July–December 2025)
During the reporting period, the project ensured sustainable access to preventive and primary healthcare services in remote and underserved communities of Poltava region.
Two multidisciplinary mobile medical teams (family doctor, nurse, psychologist, and driver) operated in coordination with Primary Health Care Centers (PHC) and local authorities. Services were delivered in line with evidence-based clinical protocols approved by the Ministry of Health of Ukraine and WHO recommendations.
Services provided:
3,275 primary healthcare services
709 psychological support services
1,167 ECG examinations
311 cholesterol tests
625 blood glucose tests
190 home visits
206 services provided to internally displaced persons (IDPs)
The project focused on vulnerable groups, including older people, persons with disabilities, internally displaced persons, and residents of remote settlements with limited access to healthcare.
Plans for 2026 (February–November 2026 | 10 months)
The project will continue operating two multidisciplinary mobile medical teams integrated into the primary healthcare system of Poltava region.
Key priorities include:
Early detection and management of noncommunicable diseases (NCDs)
Prevention of complications and improved treatment adherence
Provision of primary psychological and psychosocial support
Health education on risk factors and healthy lifestyles
Monthly monitoring and analytical reporting
Target groups:
Persons with disabilities
People aged 60+
Women and children
Internally displaced persons
People experiencing homelessness
Individuals with chronic NCDs
People affected by war-related psychological trauma
Planned indicators for 2026:
400 mobile visits (40 per month)
2,000 individuals reached (approx. 200 per month)
1,200 psychological consultations
Even in contexts of limited funding and ongoing humanitarian challenges, we remain committed to maintaining access to essential health services for vulnerable populations in underserved communities.
