Dear Partner,

Your generous support has enabled us to make important strides in combating hypertension in the region we serve, and I’m thrilled to share the progress we’ve made together. With your support, HopeCore gave refresher training on hypertension and diabetes to 768 Community Health Promoters (CHPs) and trained 655 CHPs on hypertension. They learn how to spot signs of hypertension and diabetes, counsel patients on lifestyle changes, and monitor treatment plans effectively. This knowledge today is an investment in healthier futures.

In 2024, CHPs screened 136,728 community members. Imagine that! 136,728 lives are potentially transformed through a simple, proactive step. Of these screenings, 12,375 people were identified as potentially having high blood pressure and referred for further care at the nearest health facility with CHP follow-up. But we didn’t stop there. CHPs are following up with every one of them, ensuring they stay on track with medication and lifestyle adjustments. These are personalized healthcare activities centered around the needs of the community.

When CHPs visit homes, they go beyond screening for high blood pressure. They teach families how small changes—like reducing salt, getting active, or quitting smoking—can positively impact their health. They build trust, spread knowledge, and turn awareness into action.

And why does this matter so much? Hypertension is a silent threat. Did you know that in Kenya, nearly 1 in 4 adults have hypertension, and most don’t even know it? Globally, 1.4 billion people live with this condition, and untreated hypertension can lead to heart attacks, strokes, kidney failure, and more. Every person we screen, every referral made, and every follow-up is a step toward beating these statistics.
