We at HopeCore are thrilled to share the significant progress and achievements of our Hypertension Project, which was made possible through your generous support. This report highlights the key activities, outcomes, and impact of our initiatives aimed at combating hypertension in our target communities from January to May 2024.
The primary objective of the hypertension project is to increase awareness and knowledge about hypertension and its risks, improve early detection and management of hypertension, and enhance the patient support system for those diagnosed with hypertension.
Community Health Promoters (CHPs) play a key role in creating awareness and screening household members for hypertension. They have been provided with BP machines and batteries to support them in their work. HopeCore works in partnership with Tharaka Nithi County Health Services to equip these CHPs with the knowledge and skills needed to effectively screen their communities for this silent killer.
Key achievements and highlights
Refresher training for community health workers
HopeCore conducted a refresher training on diabetes and hypertension for 768 (96.6%) CHPs across four Tharaka Nithi sub-counties. The main aim of this training was to remind them of what hypertension is, the causes, risk factors, and ways to prevent hypertension. These CHPs were provided with reference material for hypertension readings to support them during this screening exercise. Since they were trained, they have been able to reach 51,303 household members with hypertension messages. Increased awareness and education have empowered communities to take proactive steps toward preventing hypertension.
Hypertension screening and early detection
Along with awareness outreach, blood pressure screening is critical to reducing hypertension in the community. Trained CHPs screen household members using a digital BP machine to identify individuals at risk, provide immediate referrals to those with high blood pressure, raise awareness on the importance of blood pressure monitoring, and collect data to inform and improve our hypertension program.
In this period, we screened 65,658 clients and we found 5,848 individuals with high blood pressure. Early detection and management have led to improved health outcomes for those diagnosed with hypertension. Many patients report better blood pressure control and overall well-being.
