Integrating Traditional Healers at the Heart of a Sustainable Community Care System

First and foremost, please allow us to express our deepest and most sincere gratitude. Your trust, your generosity, and your commitment to our side form the very foundation of everything we accomplish every day in the Couffo department. Without your invaluable contributions, we could never have brought relief, dignity, and hope to the hundreds of patients whom the conventional health system had abandoned.

It is with genuine emotion and a heightened sense of responsibility that we present this progress report to you today. It is the living testament to what your donations have made possible — and the roadmap for what, together, we can still build.

1. A New Strategic Phase: Reaching a Decisive Milestone

Our project is now entering a pivotal phase, one that is both more ambitious and more deeply rooted in field realities. Thanks to your previous donations, we have demonstrated that it is possible to successfully treat wounds once considered incurable, restoring dignity and quality of life to forgotten patients.

Bolstered by these compelling results, our vision is now expanding: it is no longer just about treating the most desperate cases, but about tackling the root of the problem — the common illnesses that, due to a lack of appropriate and accessible care, daily degrade the living conditions of the most vulnerable populations in our region.

To achieve this, we have invested in acquiring new, cutting-edge medical-technical equipment and structured our approach around a strategic partnership with frontline actors: traditional healers, including birth attendants and bone-setters. This report details the profound human, scientific, and ethical reasons that make this choice not only justified but vital.

2. Local Context: The Health Reality of Couffo

To understand our approach, one must first understand the territory. In the rural areas of Benin, and particularly in the Couffo department where our NGO operates, the official health system faces a deep and persistent structural void.

Modern health centers are distant, often poorly equipped, and their costs remain prohibitive for a population the majority of whom live on less than one dollar a day. In this context, the traditional birth attendant/bone-setter is not merely a folkloric alternative: for thousands of women, mothers, and children, she is the first and sometimes the only available health recourse.

To ignore or ban this role without offering a credible and accessible alternative would be not only unjust but dangerous. This is why integrating these practitioners into a supervised framework of structured traditional medicine represents a historic opportunity to save lives — provided we give them the tools, training, and recognition they deserve.

Where modern medical structures are hours away on foot, costly, and often lacking essential personnel or medicines, the traditional birth attendant/bone-setter remains rooted in the heart of her community. She is, in the most concrete sense, irreplaceable in this fragile health ecosystem.

- Vital Proximity: She lives within the village itself, available at any hour, day or night, without appointments or waiting times.

- Adapted Cost: Her fees are calibrated to the economic reality of the poorest rural households. Where modern medicine excludes by price, she includes through compassion.

- Safety Net of Last Resort: For families who literally have no other option, her existence is not a luxury — it is a matter of survival.

B. Clinical Expertise Rooted in Generations of Practice

The term "bone-setter" does not denote a random practice: it embodies a real skill, inherited and refined over generations, in treating the most common musculoskeletal pathologies in the farming environment.

- Effectiveness Confirmed by Experience: For sprains, dislocations, back pain, and simple fractures related to agricultural work and heavy lifting, her manipulation techniques, combined with the use of local anti-inflammatory plants, offer rapid and lasting relief.

- Maternal and Perinatal Health: In her capacity as a birth attendant, she accompanies pregnancies with a cultural sensitivity and emotional closeness that outside medical staff cannot always provide. She is a safeguard against complications and the first blessing of every birth.

C. The Trust Capital: An Irreplaceable Social Bond

In rural communities, illness is never merely biological; it is social, emotional, and sometimes spiritual. The outside doctor may treat the body, but the traditional birth attendant/bone-setter treats the whole person.

- Therapeutic Adherence: A patient followed by someone from their community, who speaks their language and understands their beliefs, adheres far better to their treatment — which directly improves clinical outcomes.

- Early Detection and Referral: Thanks to this unique trust relationship, she is ideally positioned to detect warning signs and refer complex cases in a timely manner to appropriate hospital facilities.

- Cultural Bridge: She is the natural interface between traditional and modern medicine. By training and integrating her, we are not replacing the health system — we are complementing and strengthening it.

The stakes are clear: our approach chooses a different path — that of supervision, training, and responsible integration. This is an ethical, humane, and deeply pragmatic position.

c- Institutional Advocacy: Conduct structured advocacy with local and departmental authorities in Couffo, presenting measurable data on the effectiveness of our integrated model, to obtain its institutional recognition.

d- Support for the Most Needy: Partially subsidize the care provided by our partner practitioners for the most critical cases and the most destitute patients, so that no one is left without care due to lack of means.
