Promote Healthy Communities in Kenya: CHW Program

When someone leaves the hospital, it is easy to believe the hardest part is over. But for many patients, that's when recovery really begins.

At home, fear, uncertainty, and questions begin to fill their minds: Am I taking my medicine correctly? What if my condition gets worse? What if I cannot afford to return to the clinic? For some, medication schedules become confusing. Others miss follow-up appointments or lose hope, and without someone to walk alongside them, even a small setback can quickly become a serious health problem.

That is where Community Health Promoters step in.

Because of your continued support, HopeCore is ensuring that patients are not left to recover alone. We are empowering Community Health Promoters (CHPs) to bridge the gap between hospital care and recovery.

Living in the communities they serve, CHPs are trusted neighbors who understand the realities families face and speak the local language. Through regular home visits, they monitor recovery, ensure medications are taken correctly, encourage follow-up appointments, answer questions, and identify warning signs before they become serious. Just as importantly, they calm fears, restore confidence, and remind patients that healing takes time and that they do not have to face it alone.

Every visit brings more than healthcare; it brings hope.

Since January, we have educated 385 CHPs through empowerment and refresher training, equipped 339 CHPs through our digital classroom, conducted 173 supportive supervision sessions, and supported CHPs in reaching 112,584 households, ensuring more families receive the consistent support and care they need.

For Doris, one of the CHPs trained by HopeCore, every visit is more than a routine check; it is a chance to restore hope.
"Every follow-up visit matters because it's part of healing. When I go back, families know someone cares. I can see how they are doing, encourage them to keep going, and make sure no one is left behind," says Doris.

Whether she is checking that an expectant mother attended her antenatal appointments, monitoring a child's recovery, or visiting a patient after a clinic appointment, Doris knows that healing does not happen in a single visit. It grows through consistency, encouragement, and trust.

Karen experienced that care firsthand.

"When I came home from the hospital, I felt afraid because I didn't know how my recovery would go. Doris kept coming back every morning and every evening. She encouraged me, made sure I was taking my medicine, and gave me hope. Her visits reminded me that someone cared, and that gave me the strength to keep fighting. Today, I am healthy because she never stopped believing in my recovery," says Karen.

Karen's story is just one example of the hope and healing made possible through consistent follow-up care. Research shows that patients who receive ongoing support are more likely to stay on treatment, recover well, and achieve better long-term health outcomes.

At HopeCore, we are committed to strengthening CHPs' work through ongoing training, mentorship, and support. However, as more families seek follow-up care, the need for well-trained and equipped Community Health Promoters continues to grow.
