Dear Supporters,
We’re thrilled to share the progress your support has made on our Community Health program in 2024. Thanks to your support, Community Health Promoters (CHPs) have continued transforming health outcomes across Tharaka Nithi County. Let me share some of the highlights from 2024 that you helped make possible.
HopeCore implemented a capacity-building program for CHPs. This program equips CHPs with the necessary knowledge and skills to effectively diagnose and manage common health conditions in their communities. Through targeted training, we expanded the expertise of CHPs in the region with 1,537 trainings on topics such as TB, pneumonia, hypertension, recognizing danger signs in children, diabetes, and eye care.
Additionally, we identified areas for improvement through ongoing support supervision and conducted refresher training on diabetes and hypertension to ensure CHPs retain critical knowledge and deliver quality care. We gave refresher training to 768 CHPs on diabetes and hypertension and 719 CHPs on eye care after identifying gaps during supervision conducted by HopeCore community health staff.
The CHPs are eager to expand their expertise, requesting future training on cancer, first aid, ENT care, dental health, malaria, and Gender-Based Violence (GBV). Their hunger for knowledge and commitment to serving their communities are inspiring.
CHPs screened 136,728 community members for hypertension and referred 12,373 suspected cases for quality care. They also conducted growth monitoring on 284 children. Here’s where MUAC (Mid-Upper Arm Circumference) comes in—it’s a simple yet powerful tool for identifying malnutrition. A red MUAC signals severe malnutrition, while yellow suggests moderate malnutrition. Of those monitored, 279 children had red or yellow MUAC, prompting immediate follow-up and care with a nutritionist or with a health care facility referral.
The impact of HopeCore’s Community Health program can best be understood through the personal stories of the community members that CHPs serve. One story is from Nancy, a woman from Igambango’mbe sub-county, whose life was saved by the timely intervention of a HopeCore Community Health Promoter.
Nancy, a widow and sole caregiver for her four grandchildren, was facing overwhelming emotional and financial struggles. Battling high blood pressure and contemplating ending her life, she was visited by CHP Margaret Muthoni during a routine health visit. Recognizing Nancy’s mental health challenges, Margaret provided counseling, emotional support, and education on mental health. With Margaret’s continuous support, Nancy regained hope and strength, ultimately finding the motivation to continue caring for her grandchildren and now lives a healthier, happier life.
Listen to Nancy’s story of the impact her CHP had on her mental health and well-being on Transforming Lives: HopeCore's Podcast.
As we look ahead to 2025, we are filled with hope and excitement for the impact we can continue to make—together. Thank you for being part of this journey!
Wishing you a joyous holiday season and a prosperous year 2025.
Sincere gratitude,
Community Health Team,
