Building Toward Equilibrium: 2025 Year-in-Review

Thank you for your continued support of the Coalition for the Homeless of Houston/Harris County and our shared mission to build a homeless response system that meets our community's needs in real time. As the lead agency for The Way Home, CFTH continues working toward Equilibrium, a future where housing availability, essential services, and sustainable funding ensure that no one sleeps outside while awaiting help, and people can resolve their homelessness within 90 days.

Because of you, Houston continues to make meaningful, measurable progress toward this vision. 2025 was another year of accomplishments for Houston’s Homeless Response System, thanks to generous support from the community. Now that we have officially closed calendar year 2025, we’re pleased to share a comprehensive update capturing the work and progress made toward Equilibrium across the Greater Houston area.

Project Updates

Progress Toward Equilibrium in 2025

  • Over 350 people moved from encampments into housing paired with ongoing supportive services, helping individuals stabilize and enter a long-term housing solution.
  • Training for law enforcement partners was expanded to strengthen engagement and improve connections to housing, health care, and crisis response resources.
  • Eight new strategically selected projects with partners in The Way Home, increasing our system’s capacity to provide housing, essential services, and targeted support essential to reach equilibrium.
  • More than 2,000 people transitioned from emergency shelters into housing, reducing shelter recidivism and creating a more efficient path to stability.
  • Rapid Rehousing programs served over 3,200 individuals, helping people resolve their homelessness quickly through rental assistance and short-term case management.
  • Strengthened diversion efforts, including partnerships with eviction prevention providers, United Way of Greater Houston 2-1-1, and community organizations, helping reduce the inflow of individuals and families entering homelessness altogether.

Success Story - Connecting the Unconnected To Equilibrium 

As Houston works toward Equilibrium, removing barriers to stability is essential. In 2025, one of the most significant obstacles we identified was that many individuals experiencing unsheltered homelessness were not receiving a Coordinated Entry (CE) assessment, the critical first step to accessing housing and services within The Way Home’s Homeless Response System. Findings from the 2025 Point in Time (PIT) Count confirmed this gap, showing that many unsheltered households remained entirely disconnected from the system, limiting their opportunities to begin a pathway to housing. This insight prompted immediate action.

In response, CFTH launched an innovative partnership with Moving Waters Houston, a nonprofit dedicated to providing hygiene services to unhoused individuals. Moving Waters operates at five sites across Greater Houston, many of them “resource deserts” where traditional outreach and case management are limited. Through this collaboration, our Coordinated Entry and Outreach Teams are now present at these locations, offering CE assessments and connecting individuals with the services they need. This approach ensures that people who might otherwise remain invisible to the system have a clear and immediate pathway to stability.

This work moves Houston closer to a future where every person experiencing homelessness has a clear, accessible, and equitable path to housing, an essential step toward achieving Equilibrium.
