Helping Houston Be One Step Closer to Equilibrium

Progress You Made Possible: Sustaining a Stronger Homeless Response System

Thanks to your continued support, Houston is moving closer to Equilibrium—a future where housing availability, essential services, and sustainable funding ensure no one sleeps outside while awaiting help, and everyone has a path to stability within 90 days. Since our last update, your generosity has helped expand proven strategies across Midtown and strengthen the partnerships that make quick, compassionate housing possible.

System Progress and Innovation
Thanks to you, Houston is connecting hundreds of people to safe housing every month. Our homeless response system is becoming more agile, connected, and effective at linking people to the help they need.

  • Safe Shelter through Partnership: In August, an average of 22 people per night found a safe place to sleep at the Houston Recovery Center (HRC). Over the month, 74 individuals stayed overnight, and nearly half (36 people) were connected to housing referrals or resolutions.
  • Connecting People to Homes: Since launching new outreach initiatives, more than 185 people have been connected to housing solutions or clear pathways off the streets.
  • Targeted Outreach in Key Areas: In July and August, 81 people were connected to housing solutions through coordinated outreach across Midtown, targeting high-traffic areas including Beacon/Fannin-Prairie and Pierce/McGowen.
  • Street Multi-Access Response and Treatment (SMART) Team: This new, multi-disciplinary outreach initiative unites mental health professionals (The Harris Center), medical providers (Healthcare for the Homeless–Houston), substance use specialists (the Sobering Center), and homeless services experts (CFTH). The SMART model bridges a longstanding gap between health and housing systems, offering a proactive, person-centered approach to crisis care.

Signs of Progress: Teams are seeing fewer large encampments and reaching individuals earlier in their journey — proof that Houston’s proactive approach is working.

From the Streets to Safety at 93

In January, a 93-year-old widow found herself without housing after a disagreement over the level of care she needed. Instead of receiving the support she deserved, she ended up in a cold weather shelter and soon after, on the streets. 

That’s when the SMART team stepped in. From the moment she arrived at the shelter, the team began working tirelessly to find a safe and appropriate housing solution. Her age and unique needs meant that many standard options weren’t a fit, but the team didn’t give up. 

Just as a housing opportunity became available, we learned she had been hospitalized after being mugged, a heartbreaking setback that left her shaken and mistrustful. 

But thanks to the strength of our collaborative network, Healthcare for the Homeless Houston (HHH) maintained their connection with her. They gently rebuilt trust and encouraged her to visit an apartment offered by our housing partner, Endeavors. She loved it. 

Today, she’s safely housed in her own apartment. CFTH, The Harris Center, and Endeavors continue to visit regularly, and she’s still attending her appointments with HHH. 

Looking Ahead

CFTH is now entering the next critical phase of Equilibrium, expanding beyond the city center into the greater Houston region. Our next steps include:

  • Extending Outreach: Expanding into unincorporated Harris County and other underserved areas to connect individuals to housing and services locally.
  • Preparing for Local Hubs: Conducting feasibility studies, engaging stakeholders, and developing operational and staffing plans for regional service hubs.
  • Strengthening Partnerships: Formalizing collaborations with poverty-prevention organizations to ensure coordinated, accountable interventions.
  • Scaling Innovation: Expanding and piloting new programs, including rapid resolution and eviction-prevention strategies, mobile domestic violence advocacy, employment readiness supports, and additional SMART Teams.

Together, we’re proving that homelessness is solvable, and that Houston can lead the nation in building a future where everyone has a safe place to call home.
