Since we last spoke, HOPE Centre staff have been busy providing services and education for those at risk of or living with chronic diseases in South Africa.
Through its community outreach efforts in Zandspruit and neighboring towns, HOPE Centre staff conduct regular home visits to support and care for patients like the man pictured below who has gone blind as a result of his untreated diabetes. Under the care of the HOPE Centre, he is now receiving regular medical attention and follow-up care including the provision of his medication as needed. Adhering to a drug regimen can be difficult, especially with limited access to health care and education, yet it is critical for the management of diseases like diabetes. With support from HOPE Centre staff this man is taking his medicine and has developed strategies to distinguish between the different kinds of medication through feeling the packaging and storing them in different locations.
The HOPE Centre also recently launched the 5 Steps to Self-Care curriculum as a way to strengthen education for patients and health workers on diabetes and hypertension. Currently 19 patients divided among three groups are enrolled in curriculum and learning how to identify and modify risk factors, make healthier lifestyle choices and take advantage of available health care services through the HOPE Centre while gaining an understanding of the importance of taking medication regularly and as prescribed. The curriculum is taught using games in peer support groups to create a fun, dynamic environment for learning and encouragement.
