We provide free care to uninsured adults with diabetes and hypertension. Medications and lifestyle education help keep our patients out of the hospital, and our volunteers give them hope.
Serious complications, job loss, costly hospitalizations and even death can result when a patient is unable to obtain treatment for chronic health conditions such as diabetes and high blood pressure. For hundreds of uninsured North Portlanders, our health center is literally a lifesaver. We provide basic treatment for these conditions at no cost, allowing patients to get back to work and caring for their families. In addition to health, our volunteers give hope to patients who had given up.
North by Northeast Community Health Center provides physician visits, medication, lab monitoring, care coordination, health and lifestyle education, all at no cost to our uninsured patients.
Providing care to uninsured adults with chronic illness allows an entire community to be healthier and more prosperous. With their health under control, our patients have been able to get jobs or return to school, breaking the cycle of poverty.
