Service Expansion Using Wireless Tablets

by Community Education Group

Community Education Group provides an essential HIV/HCV and substance abuse community outreach model reaching underserved African American populations in Wards 6, 7, and 8 of Washington, DC with assessment, testing and linkage to treatment. Our work increases access to services and health outcomes for this population. CEG provides services to increase awareness, prevention, testing and access to HIV/HCV and substance abuse services for the most vulnerable and hardest hit DC residents.

The disparities of health outcomes between Wards 6, 7, and 8 and DC as a whole are glaring. These communities suffer from higher rates of chronic disease; have higher rates of premature death from chronic disease; and higher rates of HIV/HCV infection. These communities are predominately African American and have limited access to health education, screening, and medical services. Providing services to increase health literacy and access to health care will benefit Ward 6, 7, and 8 residents.

The Affordable Care Act will allow many persons to have access to health insurance for the first time. It will be critical to increase the most vulnerable communities health literacy and capacity to use systems in a preventive manner rather than an urgent care. The highlighted project will support expansions to CEG's web based data collection system and support software upgrades to increase CEG staff capacity to provide services that increase insurance enrollment and connection to medical homes.

Long-Term Impact

Over the long-term the highlighted project will increase health outcomes for under served African American populations in Wards 6, 7, and 8. The project will impact thousands of Ward 6, 7, and 8 residents by increasing health literacy and overall health awareness in the community, identifying and addressing individual health issues, decreasing amount of uninsured community members, providing health screening, linking community members to medical homes for treatment and preventive medical service
