Application Materials List for Individuals

  1. Disbursement Information
    You will be asked to provide your Social Security Number and mailing address for the Pepsi debit card.
  2. Affidavit and Grant Agreement
    To be reviewed, completed, and electronically signed.
  3. Anti-Terror Certificate
    To be reviewed and electronically signed.
  4. Non Discrimination Policy
    To be reviewed and electronically signed.
  5. Pepsi Refresh Project Terms & Conditions
    To be reviewed and electronically signed.
  6. Partner Organizations
    Are you a partnering with another organization to implement your project? If so, please submit a letter from that organization (on their letterhead) stating that they are a part of the project.
  7. Pepsi Refresh Project Plan and Budget
    Your program materials tell us about the programs that you are operating to further your organization's mission. The more detailed these documents are, the better. We would like to see what you're doing, how you're doing it, who you're helping, and how long you've been running these programs.
  8. Letter of Reference
    Please submit a letter of reference. This letter can be from a teacher, boss, colleague, coach, or anyone who knows you well, and is familiar with your capabilities. The letter should detail their relationship to you and explain why they feel you are qualified to carry out the proposed project.

    The letter should include the following information:

    The certificate of registration provides documentation that your organization is registered as a charitable organization.
  7. Financial Documents
    You will be asked to submit a copy of the latest 990 Form (or audited financial documents if you have not filed a 990). All financial information should be detailed and accurate and include a thorough breakdown of programmatic spending and administrative costs. Please be sure to include sources of income
  8. Partner Organizations

    Evidence of the (i) legal status of the organization such as organizing documents, articles of incorporation, trust agreements, charter, tax classification or certification from the appropriate authorities; and (ii) proof of good standing of the organization.

    Evidence of sound financial management and solvency (which may include audited financial statements, Dun and Bradstreet D-U-N-S Number, copy of the latest tax return, etc.)
