Certifications The certificate of registration provides documentation that your organization is registered as a charitable
organization. It should be issued by a governmental office from country in which your organization is operating. Different countries have
different names for this documentation such as Certificate of Registration, Certificate of Incorporation, Trust Deed, etc.
Financial Documents You will be asked to submit financial information for the most recently completed fiscal year and a budget for
the current year. All financial information should be detailed and accurate and include a thorough breakdown of programmatic spending and
administrative costs. Please be sure to include sources of income.
You will also be asked to provide information about your organization’s sources of funding.
Organizational Documents You will be asked to upload a scanned copy of one of the following organizational documents: Articles, M0U,
Charter, By-Laws, etc. The document should have the stamp of a government agency. If it does not then please upload a recently notarized copy.
This document must include a dissolution clause.
Staff & Board Organizations will be asked to provide the names of senior staff and board members.
Program Materials Your program materials tell us about the programs that you are operating to further your organization's
mission. The more detailed these documents are, the better. We would like to see what you're doing, how you're doing it, who you're helping,
and how long you've been running these programs.
Letter of Reference The letter of reference should come from someone who is familiar with your work and who has seen first-hand
your organization's operations and impact. In general, we recommend that you request references from funders, collaborating organizations,
prominent community members, or beneficiaries, among others. Please do not submit letters of reference from a staff, board members, family,
or individuals involved in the implementation of programs.

Disbursement Form You will be asked to select a disbursement method (check or ACH) and provide the corresponding information.
IRS Letter You will be asked to upload your organization's IRS determination letter that designates your organization as
a 501c3.
Recent 990 Form You will be asked to upload your organization's most recently completed 990 form.
You will also be asked to provide information about your organization’s annual operating budget and sources of funding.
